" The purpose of this form is to help you to clarify your thoughts about your surgery, and to allow you to express any special hopes or concerns you may have. Developing a strong understanding of exactly what you desire (and why you desire it), as well as what you don't desire, enables me to understand you better, which helps us to work together to achieve the result you want.

While the Personal Preference Form helps you think about the main issues, please feel free to add your additional comments or thoughts.

• If there are any sections you would rather not complete, please don't feel pressured - we understand that not everyone needs or wants to answer all sections. The form is designed to be helpful, and not to cause stress.



• Also, don't be concerned if your answers change as you approach surgery. For many patients that is part of the process.
